Well...I requested my records from one of my docs at UCLA and ONLY ONLY ONLY just got back the patient notes. Which these patient notes are "ONLY" what I'd told him! Nothing of "his" evaluation, diagnosis, or "his recommendation for me to see a psychiatrist either was listed.
Any suggestions?

Have a doctor/practitioner make the request and specifically ask for the stuff you didn't get.

Toni,
Under HIPAA rules you have uncontested rights to your records. The entire records.. including doctor's notes, any labs, your vitals, prescription records, diagnosis, etc., in short everything. This includes your doc's typed and hand-written notes. You don't need a third party to intervene.
Send him another request stating that you want a copy of each and every piece of paper in your files... even if they are blank forms. Don't let them intimidate you. Use the word HIPAA and state forcefully (you're good at that) for them to send your entire record this time. Let the doc's office know that you were very displeased with the first set of records which was incomplete.
It's possible that your records never got typed up and are still in the doc's handwriting. Maybe that's why they didn't send it. No matter... those are your records. They have the right to charge you a nominal fee for copying them, but they cannot deny you of these records.
By the way, if this UCLA doc was not a psych-doc and yet he made some "mental" diagnosis on you such as anxiety disorder, delusionary, etc., you can also request (no you must request) that any such statements be removed from your medical record, since those type of diagnosis are not within the scope of your doctor's field of expertise. This is important, because down the road, such comments can come back to haunt you.
PS This type of info was what KJ used to do at work... so she knows what she's talking about real well on this issue. Good luck and by all means, send that request ASAP.

Thank you Jose! hahah I laughed when I got to the part (that I'm good at) ;D
And today was on the phone with the pharm, that he'd given me a script for back on 10-06 (which was Eurax) and the pharm has the info.
A bit contracting he was, prescribed me Eurax, and tell me "it's all in my head and I'm making the sores to myself".
He told me he had good news and bad, when I saw him. He said the good news is I can help you get better. The bad news is "he said" you're not going to like what I recommend.
Which then the part about the psych came in. He even offered to get me one, and said if ( I ) called them to ask them if they're familar with "the syndrome" he called it.
I laughed sarcastically and said RIGHT! You're going to tell me my husband here has the itchy bites on his arm too "BECAUSE I'M delusional?"
He said (I've seen this happen to hundreds of families where the entire family has the same thing) *I'm sure he has too*
Oooops...tangent. Sorry. I get my hairs raised thinking about that.

Toni........I know how to make sores right in the middle of my back in places I can't even reach......so then then they starting giving T the eye like he made them...bastards....I was thinking about you in Vegas and that Chris Angel mind Freak guy and wondeing what he would say when we show him How we can oull fibers right out of our skin.....maybe he would hire one of us...
